Our doctors, together with a Nurse Case Manager, can do house calls to assess patient’s medical concerns and manage them accordingly.

We have a team of doctors that specialize in Palliative and Hospice Care, Internal Medicine, Pulmonology, Cardiology, Gastroenterology, Neurology, Geriatrics, Surgery, ENT-HNS, Nutrition, General Pediatrics, Rehabilitation Medicine.

Our Nurse Case Manager is the designated coordinator who facilitates requests of the patients and aids in executing the orders of their doctor, to ensure a smooth patient experience.

Nurse/Caregiver Deployment

Our nurses are trained in providing round-the-clock medical care for patients and coordinating with their doctors for medical management. They can also do certain nursing procedures such as administering IV and oral medication, pigtail catheter care, colostomy care, percutaneous endoscopic gastrostomy (PEG) tube care, tracheostomy tube care and more.

Our caregivers can be companions for your patients, with specialized training in palliative care. They help patients in performing their daily activities such as feeding, using the bathroom, or taking oral medication.

Initial Assessment and Discharge Planning

Initial Assessment and Discharge Planning are for patients who are currently admitted in the hospital and want to continue their care at home. Our Nurse Case Managers conduct an initial assessment of the patient in the hospital and an ocular visit in the patient’s home to explain the services of ActivCare and come up with a list of services, supplies, and medical equipment that may be needed to ensure a smooth transition of care from the hospital to home.

Outpatient Antibiotic Therapy

Our registered nurses can administer intravenous (IV) antibiotics to patients in the comfort of their home. This is usually done for patients who are admitted at the hospital for IV Antibiotic Therapy, but wish to complete their treatments at home.

Patient and Family Teaching

We provide teaching sessions with the patient and family to instruct them how to properly monitor their patient, to identify signs of pain or discomfort, and do other basic procedures that can help make the patient feel more comfortable.
