What is ActivCare?

We use a multi-disciplinary approach with a wide range of health care services that can be given in your patient’s home for an illness or injury. Home health care is usually less expensive, more convenient, and may be as effective as the care you get in a hospital.

We treat patients suffering from chronic and life-threatening illnesses who need continuing care at home, with special emphasis on Palliative and Hospice Care.

We are here for those who are suffering from Cancer, Dementia, Respiratory Diseases (COPD, Pneumonia), Cardiac Disease (Congestive Heart Failure), Liver Disease, Chronic Kidney Disease (CKD), degenerative conditions and other significant deterioration related to aging.

ActivCare provides patient and family-centered care that optimizes quality of life by anticipating, preventing, and treating suffering by addressing the physical, emotional, social and spiritual needs.

We provide services that ensure that the end of life experience is peaceful for both the patient and the family.

We provide specialized medical care for older adults, 60 years old and above, by promoting their quality of life through nurturing healthy aging, preventing diseases, treating acute illnesses and managing disabilities.

Our multidisciplinary approach ensures that all medical, psychological and daily needs of our geriatric patients are addressed holistically.

We have different programs that cater to the health and wellness of the entire family, with focus on prevention of disease through regular health screenings, acute management of sudden illnesses, and management of chronic diseases.

We also offer other programs, such as our COVID program that can manage and coordinate care of patients with mild to Severe disease, with our team of internists, pulmonologists, palliative specialists and nurses; and Nutrition programs that create personalized nutrition assessments, diet plans, and progress reports for patients of who suffer from malnutrition.

We deliver a variety of support to patients and families in the comfort of their homes.
