Geriatric and Chronic Disease

Our Geriatric and Chronic Disease program is a specialized medical care centered on older adults, 60 years old and above. Its goals are in line with ActivCare’s thrust in promoting quality of life by nurturing healthy aging, preventing diseases, treating acute illnesses, and managing disabilities.

Our multidisciplinary team composed of geriatricians, internists, neurologists, nutrition specialists, palliative specialists, skilled nursing team, and therapists ensure that the medical, psychosocial, and daily needs of our geriatric patients are holistically addressed.
